LIABILITY ACKNOWLEDGEMENT: I agree to waive any and all claims for loss, damages, injuries, etc. which I or my child may have or which may hereafter accrue to me or my child against Austin Cotillion, its owners,  instructors, officers, directors, employees, agents, independent contractors, and other representatives including the venue relating to my child’s participation in the cotillion training (Including but not limited to, claims regarding facilities, events, instruction, or supervision) whether or not caused by the negligence or willful action of the Austin Cotillion. I further agree to assume and bear sole responsibility for, and indemnify the Austin Cotillion against, any and all damages, expenses and claims associated with my child’s participation in the cotillion.

COVID-19 ACKNOWLEDGEMENT: Austin Cotillion has put in place preventative measures to reduce the spread of COVID-19; however, we cannot guarantee that you or your child will not become infected with COVID-19. Further, attending any of the Austin Cotillion classes could increase your risk and your child’s risk of contracting COVID-19. I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child may be exposed to or infected by COVID-19 by attending a class at the Austin Cotillion and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand the risk of becoming exposed to or becoming infected by COVID-19 at Austin Cotillion may result from the actions, omissions, or negligence of myself and others including, but not limited to, Austin Cotillion instructors, employees, partners, and program participants and their families.

 

PHOTOGRAPHY ACKNOWLEDGEMENT: I acknowledge that throughout the class, The Austin Cotillion will occasionally take pictures and video of activities that may be used use in scrap books, in house presentations, or promotion. If desired, I may submit in writing, a request for photos of my child, taken by the staff of The Austin Cotillion, to not be included in publicly available media.

 

AUTHORIZATION ACKNOWLEDGEMENT: I represent and confirm that I am authorized to agree with the terms and conditions with respect to the child being registered. I acknowledge that by registering my student for the Austin Cotillion, I am agreeing to these terms and conditions.
